About The Position

Spectrum Healthcare Resources has an excellent opportunity for a civilian Health Promotions Registered Nurse (RN) at the Dumfries Ambulatory Care Center in Dumfries, Virginia. This full-time opportunity supports active-duty military members, retirees, and their families in partnership with Ft. Belvoir Community Hospital. Requirements

U.S. Citizenship required
Bachelor of Science in Nursing (BSN) from an accredited nursing program and/or graduate of an accredited National League of Nursing (NLN) school
Current, unrestricted Registered Nurse (RN) license in any U.S. state, territory, or the District of Columbia
Current Basic Life Support (BLS) certification required

Nice To Haves

Minimum of six (6) years of nursing experience preferred
At least two (2) years of recent experience in public health nursing, disease management, health education, or related specialty areas strongly preferred
Certification in health education or public health is highly encouraged within the first two years of employment

Responsibilities

Promote healthy lifestyle habits and wellness initiatives for enrolled patients, with a focus on individuals with chronic conditions or increased health risk factors
Conduct retrospective reviews of patient medical records to identify individuals with specific diseases, diagnoses, or conditions
Refer and connect patients with health promotion and disease management programs designed to support improved health outcomes
Document patient interactions and program participation within ALTHA
Educate providers and clinic staff on available health promotion programs and referral processes
Support patient engagement through education, coaching, and preventive care initiatives
