About The Position

South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Our mission is to improve the lives of people that face barriers to physical, mental, spiritual and social well-being. We do so by meaningfully engaging our clients and communities, ensuring equitable access to primary health care and delivering quality care through a range of evidence informed programs, services and approaches. As a leader in community health, our locally and internationally recognized community services include cutting-edge primary health services, health promotion, harm reduction, environmental health, community food centre and population-based community programs for marginalized peoples. The Health Promoter for the Mobile Health Clinic (MHC), Health Access Taylor Massey (HATM), & Women’s Clinic coordinates and implements health education, health promotion, social prescribing programs, and facilitates the integration of a health promotion focus in HATM activities as a member of a multidisciplinary team. In addition, the Health Promoter will work in the Taylor Massey and Oakridge Neighbourhoods with a primary focus on attaching community members to Primary Care either at HATM or on the MHC. The Health Promoter also works collaboratively with community groups and agencies using a community development model. This position is further expected to be available at hours that are suitable for community members and volunteers. This may mean early morning and evening hours as well as weekend time.

Requirements

Post-graduate degree from a recognized university in health, social science, adult education, or a related discipline, or a combination of undergraduate degree and significant experience
3-5 years health promotion/education experience in a community-based organization
Minimum 2 years driving experience and a class ‘G’, ‘B’ or ‘F’ Ontario Driver’s License with fewer than three demerit points
Safe and cautious driver
Work onsite, outdoors with the Mobile Health Clinic
Ability to routinely lift, push and pull items weighing up to 40 lbs.
Experience working with diverse populations including individuals who are homeless/precariously housed and/or have mental health and/or substance use issues.
Experience with using Harm Reduction approaches
Experience and knowledge of health promotion and education, community organizing techniques, and program design, implementation and evaluation
Excellent group facilitation skills, experience and knowledge of health promotion and education, community organizing techniques and program design, implementation and evaluation
Knowledge and experience with social prescription program
Experience in community outreach
Experience in community education and engagement
Thorough knowledge of community development approaches
Knowledge of community resources, particularly in the Taylor Massey Neighbourhood
Proficiency in peer education strategies
Strong verbal and written communication skills
Proficiency in the use of computer technology and various software applications
Excellent interpersonal skills
Demonstrated skills in partnership work
Respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and effectively in a multi-disciplinary team environment
Commitment to and knowledge of community-based health care and resources
Good problem-solving skills
Police clearance documentation will be required for employment
Proof of double COVID-19 vaccination status will be required for employment

Responsibilities

Coordination of Health Promotion Programming
Support service coordination, outreach and engagement between SRCHC, MHC, HATM and SRCHC programs and services to increase access and attachment to team based primary care for priority populations
Collaborate and consult with multidisciplinary staff, partnering organizations and broader community to support program planning and development
Network and work collaboratively with community groups to strengthen and expand health promotion programming, developing and maintaining formal and informal networks
Coordinate, communicate and promote the activities or initiatives associated with HATM, the MHC and all partner agencies and the community related to expanding access to primary care
Health Education
Work both at HATM and on the Mobile Health Clinic with diverse populations including individuals whom are homeless/precariously housed and/or have mental health and/or substance use issues
Support the Women’s Screening Clinic, as well as other programs or initiatives, based on identified community needs
Identify client needs and priorities and work with team members to make appropriate internal/external referrals including social prescribing as needed
Use techniques and approaches that support crisis intervention and are trauma-informed, strength-based, culturally competent, and appropriate for client needs
Support the operation of the Mobile Health Clinic on a regular basis, including driving the MHC to community locations, helping with set-up and managing the mobile clinic environment etc.
Act as frontline support to assist clients with complex mental health and social issues by referring them to appropriate resources
Collaborate with other health providers to provide a model of holistic, interdisciplinary team
Write reports, maintain files, and collect required data for program monitoring and evaluation
Identify, in partnership with community members, health related education needs and plans, develop, implement and evaluate health education programming - using various formats such as community events, group programming, use of media and presentations to large groups
Disseminate accessible health education materials, developed with community members
Collaborate with all teams at HATM to identify and provide health education to target groups
Ensure the Mobile Health Clinic is organized, clean and presentable to clients and the public
Conduct mandatory circle checks on vehicle(s), noting minor and major vehicle defects, topping up vehicle fluids (e.g. gas and windshield wiper fluid) in a timely manner.
Report to supervisor any concerns regarding the condition of the vehicle(s) which may make them unsafe to drive
Advocacy
Advocate on behalf of priority population groups and individual community members, for accessible relevant services
Collaborate and build partnerships around issues of concern to effect change
Participate in systemic advocacy initiatives directed at providing supports for clients with complex needs impacted Social Determinants of Health
