The Health Navigator's job is to provide individuals and families with the information necessary to determine which health insurance option best fits their needs and then help them enroll in their plan of choice. This role involves maintaining expertise in eligibility, enrollment, and program specifications, conducting public education activities to raise awareness about GraceMed services, and providing information and services in a fair, accurate, and impartial manner regarding Medicaid, the Affordable Care Act, and other grant-based programs. The Health Navigator will facilitate the application and selection of a Qualified Health Plan (QHP) and provide referrals for enrollees with questions, complaints, or grievances about their health plan, coverage, or determinations to appropriate consumer assistance offices or state agencies. Additionally, the role requires providing information in a culturally and linguistically appropriate manner, including to persons with limited English proficiency and persons with disabilities, ensuring accessibility and usability of tools and functions. The position also involves accomplishing additional tasks as appropriate.
