About The Position

The Health Navigator supports individuals by helping them identify available programs and local supports. This role focuses on establishing rapport, offering general guidance, and assisting people in connecting to options that address daily needs such as food access, transportation, housing‑related concerns, and general health information. Health Navigators provide practical assistance and serve as reliable points of contact. On any given day, the role may involve assisting someone understand a blood pressure reading, locate groceries, arrange transportation for an appointment, or identify an appropriate program based on their situation. The position strengthens reach to available resources and helps individuals move through processes that may feel difficult to manage.

Requirements

  • High school diploma or equivalent required
  • Experience assisting individuals from varied backgrounds.
  • Knowledge of local programs, resource networks, and available options.
  • Strong communication skills and ability to establish rapport.
  • Ability to work across multiple processes and assist individuals in understanding next steps.
  • Skills in coordination, problem‑solving, and identifying available options.
  • Ability to maintain accurate documentation and referral tracking.

Nice To Haves

  • associate degree preferred
  • Bilingual skills a plus.

Responsibilities

  • Assist individuals in identifying available programs, benefits, and resources.
  • Link people to food access, transportation options, housing‑related assistance, childcare programs, and other essential services.
  • Track referrals, clarify next steps, and ensure individuals understand how to access recommended services.
  • Share appropriate, non‑clinical information on common conditions such as diabetes, hypertension, and other ongoing conditions.
  • Build rapport through activities, home visits, and in‑person interactions.
  • Assist individuals complete forms, interpret instructions, and communicate with program staff.
  • Maintain accurate notes, referral logs, and activity records.
  • Collaborate with partner organizations, providers, and programs to address individuals’ needs.
  • Assist with arranging transportation, coordinating appointments, or locating essential items.
