This Health Navigator is responsible for supporting Afghan refugees and SIV holders in accessing comprehensive healthcare services and building client self-sufficiency in navigating healthcare systems. This is a limited-term position funded through a grant agreement until September 30, 2026, and contingent upon funding extension. The position works with the other members of the World Relief team to provide case management services, individual coaching, and connection to resources and benefits. The position focuses on creating stability and uses a strengths-based case management approach to address client needs and promote long-term self-sufficiency in accessing health care. This position operates within grant-funded programs, requiring specific services to be provided within set timeframes and appropriate documentation of all activities. The role is part of a larger integration team and is responsible for the success of their assigned cases, as determined by the client experience, program outcomes, timeliness and accuracy of reporting, and collaboration with colleagues.
