Requirements

  • Bachelor’s degree in Health Information Management. Will accept current enrollment in Health Information Management or Associate’s Degree in Health Information Management and 2 years experience in health information OR High School Diploma/GED and 4 years of experience in health information.
  • Must be an expert in state and federal laws, HIPAA regulations, and UAMS release of information policies and procedures to ensure that patient health information is protected.
  • Proof of legal authority to work in the United States on the first day of employment.

Responsibilities

  • Assisting in the coordination and indexing of outside records received via Trace, our virtual fax.
  • Ensuring that all requests are sent to the appropriate in-box of Trace.
  • Creating a referral shell in EPIC for all new requests.
  • Ensuring that scheduled appointments have outside records sent to Medical Records to be scanned to the Electronic Patient Folder (EPF) and available to providers prior to the scheduled appointment.
  • Maintain and update patient health records accurately and confidentially.
  • Enter, verify, and organize medical information in electronic health record (EHR) systems.
  • Review medical records for completeness, accuracy, and proper documentation.
  • Process requests for medical records according to privacy and authorization requirements.
  • Release health information to patients, healthcare providers, insurance companies, and authorized organizations.
  • Verify patient identity and authorization before releasing protected health information.
  • Scan, index, file, and retrieve medical documents.
  • Correct or resolve missing, incomplete, or inconsistent information in patient records.
  • Assist with medical coding, billing documentation, or insurance-related information when required.
  • Follow HIPAA and organizational privacy and security policies.
  • Communicate with clinical staff, patients, insurance companies, and other departments to obtain or clarify information.
  • Track and respond to medical-record requests within required timeframes.
  • Maintain accurate logs and documentation of record requests and releases.
  • Assist with audits, quality-control reviews, and compliance activities.
  • Protect sensitive patient information and report potential privacy or security issues appropriately.
