As a Health/Human Service Case Manager II, you will work in the Division of Social Services’ Change Report Center, a specialized team and call center where clients report changes in their circumstances. You will process these changes and determine ongoing eligibility for programs such as Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children’s Program (CHIP), Subsidized Childcare, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). You will also provide counseling, case management, and referrals for services not offered by DSS, helping meet the diverse needs of Delaware’s vulnerable and low-income residents.
