Within this role as a Health/Human Service Case Manager II, you will determine initial and ongoing eligibility for needs-based benefits, including SNAP, Medical Assistance, CHIP, Subsidized Childcare, General Assistance (GA), and TANF. You will also work closely with TANF and SNAP vendors, provide case management and counseling, and make referrals for services DSS does not provide, such as housing, employment, Child Support, and food resources. As part of a multidisciplinary team, you will use a trauma-informed approach to identify barriers and connect vulnerable and low-income residents with benefits and community resources. This role requires strong communication and problem-solving skills, knowledge of local resources and public assistance programs, and the ability to provide timely, accurate, and compassionate service while maintaining professional boundaries and accurate case documentation.
