The Birth to Three (B23) Program provides case management for medically complex and/or developmentally delayed children and their families in Kent and Sussex Counties. Responsibilities include assessing family and child needs, developing individualized service plans, preparing social histories and progress reports, participating in transition and community meetings, and supporting quality improvement initiatives. This position will transition to the Department of Education in July 2028 and is designated as a State Health Operations Center (SHOC) emergency response position.
