About The Position

Tryfacta is seeking a Behavioral Health Case Manager II for our client in Georgetown, DE 19947. This is a temporary contract assignment. The role involves performing professional social work in a psychiatric outpatient setting. The case manager will oversee and participate in assisting individuals with developmental disabilities, mental illness, or emotional disturbance to readjust and achieve a social, economic, emotional, and physical balance and personal independence. Professional behavioral health case management services include assessments, treatment planning, direct service provision, person-centered services, and management of client risk and safety. The position reports to an administrative superior and is responsible for providing comprehensive patient care services that support the integrity of the client department. The role also includes providing consultative services to other state and private agencies, as well as referring clients/patients to appropriate social service agencies.

Requirements

  • Possession of a master's Degree in Behavioral or Social Science or related field.
  • Three years' experience in case management, which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs using effective counseling or interviewing techniques, as well as other available resources such as service plans or case management systems.
  • Three years' experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.
  • More experience in the mental health field.

Responsibilities
