About The Position

The Health Hub Liaison within the Office of Rural Health supports the Health Hub Technical Assistance (TA) Program funded through a State of Iowa RFP. The Liaison serves as the primary point of contact for rural partners and Health Hubs statewide, leading engagement efforts from initial readiness assessment through full implementation. This role focuses on building strong partnerships, facilitating planning activities, and guiding Health Hubs in developing governance structures, partnerships, and operational readiness. As the program advances, the Liaison transitions to providing hands-on technical assistance to support implementation of Hub-and-Spoke care models, telehealth integration, and long-term sustainability.

Requirements

  • Bachelor’s degree in public health, health administration, social work, or related field.
  • 1 year of related experience in community engagement, healthcare coordination, or technical assistance.
  • Strong interpersonal and communication skills.
  • Excellent communication and stakeholder engagement abilities.
  • Proficiency in Microsoft Suite (PowerPoint, Word, Excel).
  • Ability to manage multiple priorities
  • Willingness to travel statewide.
  • Demonstrated experience working effectively in a welcoming and respectful workplace environment.

Nice To Haves

  • 3+ years of experience in community engagement, healthcare coordination, or technical assistance.
  • Knowledge of rural health systems and Hub-and-Spoke care models preferred.
  • Knowledge of University and UI Health Care policies, procedures and regulations.

Responsibilities

  • Coordinate and administer operational activities for the Health Hub TA Program, ensuring alignment with project timelines and deliverables.
  • Monitor Health Hub project progress, tracking milestones, risks, and outcomes across implementation phases.
  • Analyze program data and prepare reports for internal leadership and state reporting requirements.
  • Support fiscal oversight by tracking program-related expenditures, reconciling program activities with budget expectations, and identifying potential variances.
  • Advise program leadership on resource needs, budget considerations, and operational efficiencies.
  • Conduct comprehensive readiness assessments to evaluate Health Hub capacity, infrastructure, and partnership readiness.
  • Guide Health Hubs through early-stage planning, including governance development, partnership strategies, and baseline data collection.
  • Support development of sustainable Hub-and-Spoke models, including integration of telehealth and care coordination workflows.
  • Assist Health Hubs in building cross-sector partnerships with public health, behavioral health, EMS, schools, and community-based organizations.
  • Contribute to statewide strategy development for outreach, partner engagement, and long-term program sustainability.
  • Serve as the primary liaison between UI Health Care and participating Health Hubs and rural partners.
  • Coordinate and deliver technical assistance to support workflow design, evidence-based practice adoption, and telehealth readiness.
  • Facilitate stakeholder engagement sessions, kickoff meetings, and ongoing communications throughout project phases.
  • Develop and implement communication strategies that clearly articulate expectations, timelines, and program requirements.
  • Ensure messaging consistency and alignment with program goals across all outreach and engagement activities.
  • Ensure program activities and partner engagement align with UI Health Care policies and State of Iowa requirements.
  • Coordinate documentation and reporting processes to meet grant and program compliance standards.
  • Maintain complete and accurate records of partner interactions, TA activities, and program deliverables for audit and reporting purposes.
  • Evaluate Health Hub progress and program effectiveness using data-driven approaches.
  • Analyze outcomes and engagement data to identify improvement opportunities in TA delivery and program implementation.
  • Monitor and assess communication and engagement strategies, adjusting methods to improve effectiveness and partner outcomes.
  • Support continuous quality improvement efforts across participating Health Hubs.
  • Provide functional guidance and coordination across program activities, ensuring alignment and consistency in TA delivery.
  • Support project leadership by identifying barriers, escalating risks, and recommending solutions.
  • Contribute field-based insights and partner feedback to internal program planning and strategy discussions.
  • May support onboarding, coordination, or guidance of staff involved in technical assistance delivery.
  • Design and coordinate delivery of TA messaging related to workflow design, telehealth readiness, and implementation expectations.
  • Lead communication efforts during Phase 1 (planning/readiness) and Phase 2 (implementation and problem-solving).
  • Organize and support statewide learning collaboratives and training sessions to strengthen Health Hub capacity.
  • Track engagement metrics and evaluate effectiveness of messaging strategies, refining approaches as needed.
  • Build and maintain trusted, collaborative relationships with Health Hubs as their primary point of contact throughout the project lifecycle.
  • Cultivate strong partnerships with rural healthcare organizations, public health agencies, and community stakeholders.
  • Represent UI Health Care in external meetings, trainings, and collaborative initiatives.
  • Promote program visibility, engagement, and alignment across rural communities.
