About The Position

Help support coordinated, client-centered care by turning data into meaningful support for teams improving access to care in rural communities. Schedule: Monday-Friday. This position starts their day early, around 6:30 am, due to the task of getting hospital notifications to care teams. The Health Home Data Analyst (Internal title: Health Home Care Coordinator) plays an important role in supporting high-quality, client-centered care within the Health Home program through accurate data management, strong communication, and reliable coordination across teams. This position helps ensure care teams have timely, organized information needed to support individuals receiving services. By maintaining healthcare tracking systems, validating data, and generating reports, this role contributes to program effectiveness, operational efficiency, and compliance. This is a great opportunity for someone who enjoys working with data, solving problems, and supporting care teams behind the scenes. Ideal candidates are detail-oriented, organized, and motivated by meaningful work that helps improve access to healthcare services in underserved communities.

Requirements

  • High School/GED required
  • Strong communication and customer service
  • Attention to detail
  • Basic computer literacy – knowledge and experience with Excel preferred
  • Ability to follow processes and protocols

Responsibilities

  • Maintain and update healthcare tracking systems to ensure information is accurate, current, and accessible
  • Verify and cross-reference data across multiple platforms to support workflows, reporting, and compliance
  • Generate and organize reports to support care coordination activities and program tracking
  • Communicate professionally with internal teams to support collaboration and continuity of care
  • Identify and escalate discrepancies or unclear information to ensure accuracy and consistency
  • Support special projects, operational improvements, and department initiatives
  • Participate in required meetings, trainings, and organizational events
  • Perform additional duties as assigned by department leadership
