Requirements

  • High school graduate or equivalent
  • Minimum of two years’ experience in public health
  • Bilingual English/Spanish
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
  • Strong organizational skills
  • Ability to work independently, take initiative, and be proactive
  • Ability to track and manage time effectively; highly organized
  • Ability to handle confidential materials and information in a professional manner
  • Ability and willingness to provide outreach throughout targeted areas including on the street, door-to-door, in retail outlets, businesses and other venues as needed
  • Effective verbal and written skills necessary for communication with patients/clients, clinicians and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and Excel

Responsibilities

  • Perform direct community and school outreach, educational information presentations, provide referrals and support services to youth ages 12-19 years.
  • Work with youth development program staff, Choices Teen clinic and other clinic staff, school personnel and community and youth serving agencies to increase youth access to Choices Teen Clinic and Family PACT services
  • Provide basic information on health topics, Teen Clinic services and Family Pact Eligibility as directed by Supervisor to youth ages 12-19 years
  • Conduct comprehensive outreach (and/or in-reach to include street, door to door, retail outlets, schools, teen friendly business, etc.) in North County and refer individuals to program Choices teen clinic
  • Complete appropriate paperwork and tracking logs as assigned
  • Conduct basic assessments/screenings of target population
  • Complete data collection forms
  • Participate in health fairs and other community events
  • Track incentive cards
  • Plan and implement monthly teen friendly recruitment events promoting Choices Teen Clinic and access to FPACT services
  • Demonstrate familiarity with VCC programs and services, specifically teen clinic and youth development services
  • Maintain program database and conduct daily/weekly data entry
  • Attend meetings, as needed
  • Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
  • Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
  • Perform other duties as directed
