About The Position

The Health Education Specialist is responsible for the coordination, scheduling, and presentation of the Cadena de Madres educational curriculum in the form of a baby shower. This role involves conducting hospital and home visits, coordinating participant care with Driscoll Health Plan's (DHP) Case Managers, Social Workers, and Resource Coordinators, and making appropriate referrals to other community resources. The Health Education Specialist also represents DHP at various community events and meetings. The position requires maintaining utmost confidentiality, adhering to Health System and Health Plan policies, and demonstrating ethical business practices. The specialist participates in activities to support membership growth and retention, and provides education to members and the community. They also assist the Manager of Community Outreach and Health Education and Coordinator of Health Education with administrative and program implementation duties.

Requirements

  • High School Diploma or GED
  • One year work experience in social service to include program administration
  • Strong computer skills for using Word, Excel, Access, Power Point

Nice To Haves

  • Two-year degree or Associate's degree in social work or related field
  • Three years in social work or family support as an educator
  • Equivalent combination of education and experience
  • Two years equivalent combination of education and experience

Responsibilities

  • Meeting all required health education metrics in support of the Cadena De Madres Educational Program.
  • Organizing and coordinating all site locations for the baby showers, ensuring sites are representative of DHP's image and are safe and clean.
  • Establishing a baby shower atmosphere through creative set up.
  • Ensuring all member invites are processed timely.
  • Recruitment and retention of at a minimum 10 participants attending each shower.
  • Ensuring activity reports are submitted accurately and timely.
  • Maintaining accountability of all purchases and baby shower incentives and providing detailed reports.
  • Making referrals and serving as an advocate for participants as needed.
  • Developing collaborative relationships with entities, agencies, institutions, and individuals that can provide services for families.
  • Organizing activities to foster a positive relationship with social service agencies, community agencies, and Independent School Districts.
  • Participating in community meetings and forums related to child health, Medicaid managed care, health plan programs, and community health initiatives.
  • Coordination, scheduling, and presentation of the Cadena de Madres educational curriculum.
  • Conducting hospital and home visits.
  • Coordinating participants' care with DHP's Case Managers, Social Workers, and Resource Coordinators.
  • Making appropriate referrals to other community resources.
  • Representing DHP at a variety of community events and meetings when appropriate.
  • Maintaining utmost level of confidentiality at all times.
  • Adhering to Health System and Health Plan policies and procedures.
  • Demonstrating business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines.
  • Participating in various activities associated with achieving the Department Strategy to effect membership growth and retention, as well as to provide education to its members and the community.
  • Assisting the Manager of Community Outreach and Health Education and Coordinator of Health Education in all administration and program implementation duties and responsibilities.
