About The Position

This position is responsible for the supervision of staff including appraisals, planning and directing work, approving leave, and having the authority for hiring and taking disciplinary action or effectively recommending such actions. Serves in a supervisory role of the Healthy Start Program, with responsibility for daily program operations and Care Coordinators assigned to the Healthy Start Program and the Okaloosa County Health Department (OCHD). Work is performed under the direction and supervision of the Healthy Start Program Director, with strict adherence to all Florida Department of Health (FDOH), OCHD and Healthy Start policies and guidelines. Serves as a subject matter expert for Care Coordinators, providing training and guidance in Healthy Start Program guidelines and best practices. Manages caseload/client assignment for Care Coordinators and monitors and reviews productivity and performance by routinely conducting ride-along to home and offsite client visits. Assures client services are documented by Care Coordinators with accuracy and within required timeframes. Generates, develops and/or reviews programmatic reports from various systems or data sets. Monitors current client list and waitlist, number of services provided to clients, timeliness of staff documentation, and quality of documentation. Attends Healthy Start Coalition meetings as requested. Provides, disseminates and ensures utmost transparency and clarity of communication with and between Care Coordinators, Healthy Start Program Director and Healthy Start Coalition. Reports all issues or concerns to Program Director promptly. Attends routine and episodic meetings with Healthy Start Program Director to review programmatic performance and objectives and receive coaching and development. Conducts case management and provides care coordination for Healthy Start clients of Okaloosa County, including women during pregnancy and to their infants after delivery. Provides health education and care coordination to clients and their families in a variety of settings and using a variety of modalities, including but not limited to phone, video conferencing, in office/clinic, and at clients’ home, work and/or school. Records all applicable work-related mileage for personal vehicle, maintains all required travel approval/reimbursement documentation, adheres to all travel-related policies and guidelines. Reimbursement for approved work-related mileage is provided in accordance with DOHP 56-37-22 Travel Policy. Provides clients and their families with education in prenatal health, parenting skills and infant/child health, health education resources, and referral/follow-up to other providers as indicated. Provides or facilitates psychosocial assessments, ASQ's, Domestic Violence Screenings, and appropriate referrals for intervention for Healthy Start and other identified high-risk clients. Will provide a structured interaction between the professional and client which assists client to identify concerns or needs by addressing social, emotional, behavioral and environmental issues that affect the client's well-being. Develops individual client and family support plans that ensure all clients receive appropriate services and support and work to minimize obstacles to full participation in prenatal and infant health care. Documents all client activities and codes all services in the Well Families System with utmost accuracy and timeliness, and with strict adherence to Healthy Start client/service documentation guidelines. Assists in the collection of statistical data and evaluating collected information to ensure proper education and resources are provided to meet client needs. Participates in monitoring/QI-QA activities to assure compliance with program guidelines, objectives and regulations. Plans, conducts, and/or participates in organized education and outreach activities that support the Healthy Start program. Attends appropriate staff meetings, conferences and training sessions to enhance job skills and knowledge. Participates in committees and work groups as requested. Performs all other duties as assigned in support of the Agency’s mission and vision.

Requirements

  • Ability to assist in planning and conducting a variety of training activities.
  • Strong written and verbal communication skills.
  • Ability to establish and effective working relationships with others.
  • Experience coordinating and/or conducting group meetings.
  • Experience using Microsoft Office applications in a professional role (i.e., Outlook, Teams, Excel, Word, PowerPoint, etc.)
  • Personal vehicle or access to vehicle for home-based or offsite client visits required.
  • Knowledge of health education principles with a focus on maternal-child health, childbirth, breastfeeding, smoking cessation, and/or other relevant public health focus.
  • Valid U.S. Drivers’ License
  • Ability to learn and communicate effectively, orally and in writing, in English.

Nice To Haves

  • Bachelor’s degree or higher in social sciences, social work, or health related field such as nursing, health education, public health or health care administration OR associates degree/two years of college coursework and three years’ work experience in a public health or maternal-child health role.
