About The Position

Pioneer Human Services is seeking a Health Coordinator to join their team. This role focuses on coordinating and monitoring client physical and behavioral health services within residential treatment facilities. The Health Coordinator will ensure continuity of care by facilitating communication between clients, nursing staff, medical and behavioral health providers, pharmacy services, and program staff. Responsibilities include supporting the intake process, coordinating appointments, assisting with medication monitoring and documentation, and gathering health information. The role also involves direct client support, including engaging clients, observing health status, reinforcing treatment plans, assisting with healthcare access, and responding to urgent health concerns. This position requires strong organization, attention to detail, and the ability to support clients with complex needs in a structured residential environment.

Requirements

  • High School Diploma or Equivalent
  • One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting, or experience supporting marginalized or vulnerable populations
  • Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain and maintain active registration within 90 days of hire in accordance with WAC 246-810-015; RCW 18.19.210
  • First Aid/CPR certification within 90 days of hire
  • Proficient level of written/verbal communication, and efficiency with independent task management and ability to show attention to detail.
  • Basic computer skills and the ability to learn
  • Demonstrated ability to maintain a professional, empathetic, and client-centered approach when working with individuals in recovery

Nice To Haves

  • Experience with electronic health records (EHR) or electronic records management systems.
  • Proficiency in Microsoft Office Suite, Outlook, and Internet navigation.
  • Certified Nursing Assistant certification

Responsibilities

  • Coordinate and monitor client physical and behavioral health services within residential treatment facilities.
  • Facilitate communication between clients, nursing staff, medical and behavioral health providers, pharmacy services, and program staff.
  • Support the intake process.
  • Coordinate internal and external medical and behavioral health appointments (including telehealth).
  • Assist with medication monitoring and documentation accuracy in the electronic medication administration record (eMAR/MAR).
  • Gather and communicate relevant health information to appropriate clinical staff.
  • Work under the direction of nursing staff or the Health & Resource Coordination (HRCU) Manager.
  • Provide direct client support, including engaging clients in the milieu, observing and monitoring client changes in physical and behavioral health status, reinforcing treatment plans and health education, assisting with access to healthcare services, and responding to urgent health concerns.
  • Utilize program-approved screening tools and established procedures to identify and document client risks, needs, and barriers.
  • Utilize Motivational Interviewing (MI) and other engagement strategies to support client participation and skill development.
  • Interpret and explain program policies and goals to clients.
  • Conduct health education activities and reinforce self-care skills.
  • Observe and document client self-administration of medications.
  • Communicate medication-related concerns, discrepancies, or client requests to the licensed nurse.
  • Review MAR documentation for completeness at shift change and escalate discrepancies.
  • Maintain a safe and healthy environment, including cleaning and disposing of equipment and supplies.
  • Assist with medication storage, ordering, and returns.
  • Assist clients in meeting health and safety needs.
  • Provide basic first aid and serve as the first point of contact for emergency response.
  • Follow provider orders and support implementation of care plans.
  • Gather and relay client-reported health information to nursing staff or providers.
  • Assist Registered Nurses (RN) or Licensed Practical Nurses (LPN) with intake processes or coordination activities.
  • Support coordination of client physical and behavioral healthcare services during assigned shifts.
  • Coordinate and document client intake and discharge processes after hours.
  • Coordinate internal and external services and referrals.
  • Assist clients in accessing health services and communicating needs to clinical staff.
  • Maintain accurate, timely, and complete documentation of services in the electronic health record.
  • Ensure required notifications are completed during assigned shifts.
  • Complete eligibility assessments and follow-up assessments.
  • Ensure client information is accurate and updated from intake through discharge.
  • Record routine case notes and service contacts documenting client progress and services provided.
  • Review reports with the care team to identify service gaps and coordinate appropriate follow-up support.
