About The Position

As a Health Coach at Cylinder, you’ll deliver compassionate, evidence-based support to members seeking relief from chronic digestive conditions. Through HIPAA-compliant phone, video, and messaging sessions, you’ll use motivational interviewing, strengths-based care, and behavior change models to help members build healthier habits and achieve their goals. Most members you’ll work with are managing conditions such as Crohn’s disease, ulcerative colitis, celiac disease, IBS, diverticulitis, chronic constipation or diarrhea, and more. You’ll primarily support members with stress management, mindfulness, sleep, physical activity, medication adherence, self-monitoring, and self-advocacy—helping them feel confident and supported in their care. This role is designed for approximately 15 hours per week, with a primary focus on evening and weekend coverage.

Requirements

  • National Board Health and Wellness Coach (NBHWC) certification, or eligibility to sit for the exam within six months of hire
  • Bachelor's degree in a health-related field
  • 1+ year of face-to-face, telephonic, or video health coaching experience. This does not include chat-based coaching
  • Ability to work a consistent schedule that includes evenings and weekends
  • Quiet and professional dedicated workspace with secure high-speed internet

Responsibilities

  • Build trusting, supportive relationships with members, serving as a consistent partner in their health journey.
  • Apply evidence-based strategies (e.g., stress management, mindfulness, sleep hygiene, physical activity, medication adherence, self-monitoring) to improve members’ well-being and quality of life.
  • Use counseling skills (MI, TTM, strengths-based care) to help members progress through stages of change.
  • Guide members in building self-efficacy and advocacy, while providing education, SMART goal-setting, and accountability.
  • Refer members to Registered Dietitians for Medical Nutrition Therapy (MNT) in line with escalation protocols.
  • Document coaching sessions in line with program guidelines and company standards.
  • Collaborate with the interdisciplinary team to ensure coordinated, whole-person care.
  • Practice in alignment with HIPAA, state, and federal regulations, following our Clinical Pathways and scope of practice, while collaborating with or referring to other disciplines as outlined in our escalation guidelines.
  • Maintain strict confidentiality of members’ Protected Health Information (PHI) in compliance with HIPAA and company policy.
