About The Position

We’re the largest consumer-directed home care agency in NY State, and are continually expanding our areas of focus including operations in other states and tech-based innovations. This role requires fieldwork through the Atlanta, GA area. Structured family caregiving is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs, while preserving the dignity, self-respect, and privacy of the patient. The Health Coach Specialist II will be a vital part of our operations in Georgia. Responsibilities include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they provide the best service possible to their loved ones.

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers
  • Valid driver’s license with an insured vehicle for travel within the state
  • Excellent verbal and written communication, organizational and time management skills
  • Strong critical thinking and problem-solving skills
  • Proficient in Microsoft (i.e., Word, Excel, Outlook)
  • Excellent training skills
  • Team-player mentality

Nice To Haves

  • Previous field experience in a medical or case management environment, preferred
  • Experience with data input, case review/case note entry; Salesforce experience is a plus
  • Hands-on experience in home care, preferred
  • Working knowledge of ADL/ IADL assessments

Responsibilities

  • Home orientations involving signing of paperwork
  • Initial assessments during orientations
  • Periodic supervisory visits / check in for on-care cases
  • Training caregivers / patients as required per case
  • Interacting with the caregiver and other stakeholders, as needed
  • Engaging in interdisciplinary meetings with the care managers, caregivers, and patients
  • Collaborate in the care practice and communicate impressions of cases to appropriate staff
  • Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
  • Wear multiple hats as operations develop
  • Travel within the state of Georgia (Atlanta Metro Area)
