About The Position

FreedomCare is a healthcare company dedicated to revolutionizing the home care industry since 2016, supporting patients across the U.S. by empowering them to choose a caregiver for their home care needs. The company is looking for a Health Coach Specialist II for its team in Colorado. This role requires fieldwork 5 days per week throughout the Denver, CO area. Structured family caregiving is a caregiving arrangement where a patient lives with a principal caregiver who provides daily support based on the patient's needs. The goal is to provide necessary care while fostering patient independence in a home environment, offering options as needs evolve. This service provides alternatives to nursing facility-level care for individuals who meet those needs, while preserving patient dignity, self-respect, and privacy. The Health Coach Specialist II will be a vital part of operations in Colorado, responsible for performing orientations, assessing new patients for the program, and training caregivers to ensure optimal service delivery.

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers
  • Valid driver’s license with an insured vehicle for travel within the state
  • Excellent verbal and written communication, organizational and time management skills
  • Strong critical thinking and problem-solving skills
  • Proficient in Microsoft (i.e., Word, Excel, Outlook)
  • Excellent training skills
  • Team-player mentality

Nice To Haves

  • Previous field experience in a medical or case management environment
  • Experience with data input, case review/case note entry; Salesforce experience is a plus
  • Hands-on experience in home care
  • Working knowledge of ADL/ IADL assessments

Responsibilities

  • Home orientations involving signing of paperwork
  • Initial assessments during orientations
  • Periodic supervisory visits / check in for on-care cases
  • Training caregivers / patients as required per case
  • Interacting with the caregiver and other stakeholders, as needed
  • Engaging in interdisciplinary meetings with the care managers, caregivers, and patients
  • Collaborate in the care practice and communicate impressions of cases to appropriate staff
  • Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
  • Wear multiple hats as operations develop
  • Travel within the state of Colorado (Denver Metro Area)
