About The Position

Chestnut is seeking Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for positions that offer an improved work-life balance, featuring weekday schedules, ten paid days off in the first year, and eligibility for sign-on and retention bonuses, among other benefits. The role involves driving integrated care across primary care and psychiatric programs. This position will primarily be based in Chestnut's Granite City Health Center, with occasional support required in Maryville or Belleville. The typical work schedule is Monday through Friday, from 8 am to 5 pm.

Requirements

High school diploma or equivalent
Practical nurse licensed in Illinois OR registered nurse licensed in Illinois
Basic computer skills and experience to include MS Word, email, videoconferencing, e-prescribing, and entry into an electronic medical record (EMR)
Must complete required substance-use training within first week of employment, in order to be assigned to detoxification services.

Responsibilities

Carry out health care plans designed by practitioners.
Administer treatments, medications, and medical tests.
Gather patient health histories.
Assess patients for acute physical issues.
Act as a liaison among prescribers, patients, laboratories, PCPs, other providers, pharmacies, and staff to ensure continuity of care.
Provide health teaching as well as support and guidance for patients in maintaining a therapeutic environment.
Managing referrals to specialists or facilities by assessing needs.
Ensuring smooth communication between patients, providers, and payers.
Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness.
