About The Position

Under the supervision of the PACE Director and under the direction of primary care, this position is responsible for assessing, coordinating, monitoring, and providing clinical treatment and services required by PACE members. This role involves direct supervision and evaluation of the clinic team, ensuring consistent delivery of care, and managing various aspects of health center operations. The Health Center Manager RN will also serve as a liaison between providers, families, and pharmacies, and will be an active member of the interdisciplinary team, with the potential to cross-train to a home care RN position. The position requires strong organizational skills, the ability to function independently, and a commitment to maintaining a safe, orderly, and sanitary environment for participants and staff.

  • Emergency response capabilities for possible crisis situations.
  • Manual dexterity and strength sufficient to enter information via computer keyboard for long periods of time, to write notes and information as needed, and to pick up and hold paperwork, supplies and other items.
  • Speaking and hearing ability sufficient to handle phone and in-person conversations with patients/clients, clinicians and other staff and to listen to breathing, pulse and circulation, as part of patient examination and treatment.
  • Visual acuity and reading ability sufficient to effectively read records and documents, and to accurately view information on a computer monitor.
  • Normal tactile feeling required to perform duties, including taking patient’s pulse.
  • Eye/hand coordination, hearing and visual acuity necessary for driving safely.

Nice To Haves

  • Experience with an elderly population preferred.
  • Experience/familiarity with PACE program and Electronic Medical Records systems.
  • Previous Home Health experience.

Responsibilities

  • Assess, coordinate, monitor, and provide clinical treatment and services required by PACE members.
  • Direct, supervise, and evaluate the performance of the clinic team, maintaining a matrix reporting/supervisory relationship with staff involved in participant care.
  • Interview, recommend for hire, and train staff specifically for Clinic Operations.
  • Prepare periodic reports and assist with QI projects to review staff performance.
  • Establish and maintain uniform, consistent delivery of care and services for participants, including internal and external appointments with transportation needs.
  • Arrange Licensed Vocational Nursing (LVN) and Medical Assistant (MA) coverage for all areas of responsibility.
  • Ensure that cultural needs of participants are met in the day center.
  • Assist in the development and implementation of new procedures.
  • Perform functions related to clinical assessment, care planning, and evaluation of participant care.
  • Teach participants and families based on identified needs.
  • Document nursing care in the medical record.
  • Triage participants in clinic effectively.
  • Respond to emergencies involving participants.
  • Administer medications and maintain the medication system.
  • Complete transcription and follow-up of medical orders.
  • Perform phlebotomy procedures as needed.
  • Maintain cleanliness and organization in the clinic, especially in the medication area and cart.
  • Participate and function as a member of the interdisciplinary team and rotate as needed.
  • Independently deliver nursing care and services to program members within scope of licensure and per primary care orders or nursing care plan.
  • Schedule and coordinate med pass and monitor for quality review of medication errors.
  • Mentor and assist in the orientation of new clinic staff hires.
  • Oversee medical, pharmaceutical, and oxygen supplies in the clinic.
  • Track assignment of oxygen concentrators and other Durable Medical Equipment.
  • Schedule and manage appointments/assessments for MDs and Nurse Practitioners.
  • Manage wound care and maintain wound records/documentation.
  • Perform daily nursing tasks, preparing the clinic for patients.
  • Assist with bedside procedures including gynecological exams and phlebotomy.
  • Maintain participant safety while at the center.
  • Hold formal monthly and as needed meetings with PCWs and record minutes.
  • Monitor transportation services and resolve problems with the Transportation Coordinator.
  • Consult and collaborate with the Dietitian and Recreation Therapist on matters related to food/nutritional services.
  • Develop and implement policies, procedures, and standards for the Health Center.
  • Communicate changes to staff, monitor outcomes, and make changes as needed.
  • Assume a leadership role in the development, implementation, and refinement of day center systems and coordination of schedules.
  • Interface with program participants, families, contracted providers, and the public regarding health center operations.
  • Monitor the physical environment of the Health Center to ensure a safe, orderly, and sanitary environment.
  • Notify appropriate maintenance or housekeeping staff of problems or concerns.
  • Review safety issues, including participant falls and incidents, and report to appropriate personnel and committees.
  • Serve on assigned committees of the organization.
  • Assist the PACE Director with budgeting, monitoring, and analyzing site performance trends.
  • Investigate and resolve grievances, service delivery concerns, and incidents relating to Health Center operations.
  • Supervise the Health Center staff.
  • Act as liaison between providers/family/pharmacy.
  • Maintain excellent and respectful working relationships with co-workers and customers from diverse backgrounds.
