About The Position

We are seeking a compassionate and highly organized Health Care Occupational & Case Manager to coordinate and manage care for patients with complex medical conditions, chronic illnesses, or disabilities. In this role, you will work closely with patients, healthcare providers, insurance companies, and other stakeholders to ensure patients receive appropriate care, support, and resources to improve their overall health outcomes and quality of life. The ideal background is in nursing, social work, or a related healthcare field, along with experience in case management. Strong communication, organizational, and problem-solving skills are essential, as well as a patient-centered and empathetic approach to care.

Requirements

  • Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related healthcare field.
  • Experience in case management, care coordination, utilization management, occupational health, or a related healthcare role.
  • Strong knowledge of healthcare delivery systems, insurance benefits, and care coordination principles.
  • Excellent verbal and written communication skills.
  • Strong organizational, critical thinking, and problem-solving abilities.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Proficiency with Electronic Medical Record (EMR/EHR) systems and Microsoft Office applications.

Nice To Haves

  • Active clinical license (RN, LPN/LVN, LMSW, LCSW, or other applicable healthcare license), depending on position requirements.
  • Certified Case Manager (CCM) or other relevant case management certification.
  • Experience working with Medicare, Medicaid, commercial insurance plans, or workers’ compensation programs.
  • Knowledge of occupational health and disability management practices.

Responsibilities

  • Conduct comprehensive assessments of patients’ medical, psychological, social, and functional needs.
  • Develop individualized care plans in collaboration with healthcare providers, patients, and caregivers.
  • Coordinate healthcare services, including medical treatments, therapies, specialist consultations, and community resources.
  • Monitor patient progress and adjust care plans as needed to ensure appropriate and effective care.
  • Provide education and support to patients and their families regarding treatment plans, available resources, and ongoing care needs.
  • Advocate for patients’ needs and rights within the healthcare system to ensure access to appropriate services and support.
  • Communicate effectively with healthcare providers, insurance companies, and other stakeholders to facilitate coordinated care.
  • Evaluate the effectiveness of care plans and recommend improvements to enhance patient outcomes.
  • Maintain accurate and timely documentation of patient assessments, care plans, interventions, and communications.
  • Stay current with healthcare regulations, policies, payer requirements, and best practices related to case management.
  • Participate in interdisciplinary care planning meetings and quality improvement initiatives.
  • Perform additional duties as assigned.
