About The Position

The Health Care Education Coordinator will identify, access, and refer patients and families to appropriate community and support services. This role provides care navigation and logistical support following emergency room or inpatient discharge. The coordinator will develop and maintain relationships with primary care and specialty providers to ensure coordinated care, and participate in interdisciplinary care team meetings, collaborating across departments. Responsibilities include conducting outreach in the emergency room and outpatient clinic, particularly for asthma-related cases, and maintaining accurate documentation as required by the Department of Pediatrics, PCMH, DSRIP, and the Asthma Coalition of Staten Island. The role also involves assisting with data collection and entry for program tracking and reporting, and attending community-based meetings and representing RUMC at outreach events several times per month. Support for general and condition-specific initiatives related to asthma, obesity, mental health, maternal depression, and developmental delays is also a key function.

Requirements

  • Bachelor’s degree in Public Health, Health Administration, or related field required
  • Knowledge of pediatric care systems and community health resources
  • Strong communication and organizational skills
  • Ability to work collaboratively across departments and with external partners
  • Culturally sensitive and family-centered approach to care
  • Maintains professionalism, compassion, and respect in all interactions
  • Communicates clearly and responsively with families and team members

Nice To Haves

  • Master’s degree (e.g., MPH, MHA) preferred
  • Experience in pediatric care coordination or community health strongly preferred
  • Physician (MD/DO) applicants will be considered if interested in a non-clinical, coordination-focused role
  • Familiarity with EMRs and data tracking systems a plus
  • Bilingual skills (e.g., Spanish, Russian) preferred
