About The Position

The Health Care Coordinator is responsible for coordinating the health care needs of the ICF/IID clients. This role involves ensuring clients attend necessary medical appointments, scheduling these appointments efficiently, and coordinating transportation. The coordinator must maintain good relationships with healthcare providers, document all relevant information, and communicate effectively with nursing staff and guardians. Additionally, the role requires advocating for individuals' rights, ensuring HIPAA compliance, and monitoring adaptive equipment.

Requirements

  • Must be 18 years of age or older.
  • Minimum of one year of working experience with adults with intellectual and physical disabilities.
  • Ability to make independent decisions when circumstances warrant such actions.
  • Must possess a current Certified Medication Administration certificate.
  • Must have patience, tact, cheerful disposition and enthusiastic, as well as be willing to handle individuals based on whatever intellectual maturity level they may have.
  • Able to handle and lift a minimum of 50 pounds.
  • Must be able to sit, stand, and have the physical flexibility to be able to move about throughout the workday.
  • Must have an active home phone or cellular phone.
  • Must have dependable transportation.
  • Ability to drive day or night.
  • Must be able to speak and hear clearly.
  • Must be able to meet the physical demands associated with the Health Care Coordinator position.
  • Must be able to operate any passenger vehicle, including bus-like vehicles.
  • Ability to lift 50 pounds with or without accommodation.
  • Vision sufficient to read or assist individuals in normal or semi-darkened environments.
  • Ability to understand oral and written instructions.
  • Must have good written and oral communication skills.
  • Ability to sit, stoop, or stand for prolonged periods of time.

Responsibilities

  • Ensure that clients are scheduled for and attending all necessary medical appointments.
  • Schedule appointments directly with health care providers in the most efficient way.
  • Transport clients to health care appointments and/or coordinate transportation to/from appointments with RSL, Nursing or QIDP staff.
  • Ensure that the client arrives at the scheduled appointment with all necessary information and paperwork.
  • Maintain an excellent working relationship with the medical profession and other health related facilities.
  • Document appointment notes, follow-up and other pertinent information in Therap where it is accessible by staff, nursing and Administration as necessary.
  • Communicate directly with Nursing and the parent/guardian, if necessary, any relevant information from health care appointments.
  • Meet with the Nursing, RSL, and/or QIDP, on a regular scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.
  • Maintain CONFIDENTIALITY of all pertinent individual care information to assure individual rights are protected.
  • Act as an advocate in protection of individuals' rights and responsibilities.
  • Ensure HIPAA compliance in all program areas.
  • Check on individuals’ adaptive equipment and report issues to nursing.
