About The Position

This is a grant-funded position for a Health and Wellness Specialist based out of Texas Health Dallas and Texas Health Fort Worth. The role involves delivering structured group education sessions focused on chronic disease prevention, postpartum recovery, and healthy lifestyle practices. The specialist will guide participants through curriculum content, facilitate discussions, and support outreach efforts to recruit participants into lifestyle education programs. The position requires adherence to the Centering curriculum while adapting delivery to meet diverse community needs, with a focus on supporting postpartum mothers in adopting healthy behaviors to reduce long-term health risks.

Requirements

  • High School Diploma or Equivalent required.
  • MA - Medical Assistant Certified Medical Assistant (CMA) from a nationally recognized certifying organization. Required upon hire.
  • National Board-Certified Health & Wellness Coach (NBC-HWC) 18 Months required.
  • 4 Years Experience in a healthcare or community health environment providing patient education, facilitating health discussions, or presenting health-related information to individuals or groups, required.
  • CPI - Crisis Prevention Intervention Training 90 Days required.
  • Driver’s License (required)
  • BCLS - Basic Cardiac Life Support 90 Days required.

Responsibilities

  • Deliver structured group education sessions utilizing the approved THR Curriculum focused on chronic disease prevention, postpartum recovery, and healthy lifestyle practices.
  • Guide participants through curriculum content related to nutrition, physical activity, stress management, chronic disease prevention, and long-term wellness.
  • Facilitate discussions that encourage peer support, shared learning, and participant engagement.
  • Maintain fidelity to the Centering curriculum while adapting delivery to meet the needs of diverse community populations.
  • Support postpartum mothers in adopting healthy lifestyle behaviors that reduce long-term risk for chronic conditions such as diabetes, hypertension, and cardiovascular disease.
  • Support outreach efforts to recruit eligible postpartum mothers and community members into lifestyle education programs.
