About The Position

At Ability Matters, we are dedicated to supporting individuals with autism and other neurological disabilities, helping them build independence, confidence, and meaningful lives within their communities. The organization is currently expanding its team and is seeking compassionate individuals for the Health and Wellness Coordinator role. This position is crucial for ensuring continuity of care by coordinating essential medical, dental, vision, and wellness appointments. The coordinator will also be responsible for maintaining accurate, current, and compliant records, providing direct services three days a week to gain firsthand consumer experience, and facilitating smooth communication and advocacy among individuals, families, providers, and internal teams. Furthermore, the role involves connecting individuals to community resources and opportunities that promote overall well-being and participation. Requirements

  • High school diploma or equivalent required.
  • Strong organization and attention to detail.
  • Clear, professional communication.
  • Able to work independently and manage multiple priorities.
  • A genuine desire to support individuals with disabilities.
  • Valid driver’s license and reliable transportation.

Nice To Haves

  • Relevant degree (preferred).
  • Experience in healthcare coordination, scheduling, records management, or social services is a plus.

Responsibilities

  • Coordinate Essential Care: Schedule, track, and manage medical, dental, vision, and wellness appointment to ensure continuity of care.
  • Keep information Organized & Compliant: Maintain accurate records, monitor documentation, and ensure information stays current, secure, and aligned with requirements.
  • Contribute to Direct Services: Provide direct services three days per week to stay connected to the consumer experience and understand the impact of services firsthand.
  • Communicate & Advocate: Work closely with individuals, families, providers, and internal teams to ensure appointments and care plans run smoothly.
  • Support Community Inclusion: Help connect people to resources, events, and opportunities that promote wellbeing and participation.
