About The Position

Episcopal SeniorLife Communities (ESLC) is seeking a Part-Time Health and Wellness Coordinator to join their team. This role is responsible for planning and coordinating programs and activities that enhance the well-being of older adults in the community, with a specific focus on integrating dementia-friendly programs and support for individuals impacted by Alzheimer's and other dementias, as well as their caregivers. The coordinator will ensure high-quality care and customer service in all programming, promote participation in ESLC's Neighborhood Programs, and expand health and wellness initiatives through a volunteer workforce. The position requires a creative, organized individual with strong interpersonal skills and computer proficiency.

Requirements

  • Associate degree in human services or related field preferred.
  • Experience in planning and providing health and wellness activities for older adults preferred.
  • Valid New York State driver’s license with a clean driving record and willingness to drive ESLC vehicle to transport residents.
  • Evidence of creativity.
  • Demonstrated organizational skills.
  • Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors, and the general public.
  • Possesses computer skills (Microsoft Word, Office, and Publisher).
  • Ability to occasionally lift loads of 30 pounds without assistance.
  • Ability to sit, talk, and hear is required.
  • Specific vision abilities required include close vision, distance vision, peripheral vision, color vision, and the ability to adjust focus.

Responsibilities

  • Develop and deliver dementia-friendly health and wellness programming and a robust volunteer program.
  • Ensure all programming is dementia friendly and meets the highest standards of quality care and customer service.
  • Build and maintain positive relationships with community members.
  • Promote and encourage participation in all of ESLC’s Neighborhood Programs, including the Medicare GUIDE Program.
  • Expand and deliver health and wellness programs in partnership with a volunteer workforce.
  • Promote the program through a tailored balance plan, nutrition education, strength training, informative presentations, and other activities.
  • Ensure all programs have a measurable impact on participants through qualitative and quantitative assessments and surveys.
  • Ensure all programs are inclusive for all communities residing within each neighborhood.
  • Develop and deliver educational programs addressing all aspects of healthy living through seminars, lunch and learns, and trips to community events and programs.
  • Recruit, supervise, and coach a team of volunteers to deliver programs in a culturally competent manner.
  • Monitor volunteer impact and take action to improve volunteer performance.
  • Represent ESLC at networking, recruiting, and tabling events in collaboration with the ESLC outreach team.
  • Complete all health and quality care assessments as required by funders.
  • Make adjustments to the program to ensure high participant satisfaction and impact.
  • Track and report necessary information/data to fulfill grant requirements.
  • Drive ESLC vehicle to transport residents.
