Requirements

  • AA degree in related field
  • 3 years of relevant management, progressive supervisory experience, and staff development experience.
  • Ability to create and deliver curriculum and staff development material.
  • Two years of experience working with marginalized populations.

Responsibilities

  • Overseeing the coordination of services, including internal and community-based medical and behavioral healthcare, transportation, records requests and storage, education and employment services, case management, discharge planning and aftercare services and referrals.
  • Supporting program development, operation, and quality assurance by tracking/measuring outcomes, reviewing policies, and ensuring compliance with contracting entities and all governing regulations.
  • Ensuring coordination of all resident physical/behavioral healthcare services on your unit, including telehealth, clinic, outside/visiting provider appointments, transportation, and educational services.
  • Ensuring high level of cross-departmental communication and collaboration for continuity of care.
  • Ensuring proper policy and procedures, forms and other materials needed to consistently perform the functions of the unit are in place, up-to-date, and reviewed bi-annually for relevance.
  • Ensuring all staff are trained in current policy and procedure and can perform to expectations.
  • Adding, evaluating, or changing services as needed to meet the changing needs of the population and organization.
  • Maintaining quality of care consistent with or exceeding standards set by Washington State.
  • Collaborating with community partners to ensure mutual coordination of care needs are addressed.
  • Implementing and/or approving program policies and procedures as directed and consult on the development and improvement of policies and procedures that maximize the safety and welfare of the residents, staff, and visitors to the program.
  • Ensuring compliance with all regulatory guidelines.
  • Implementing necessary quality and inventory control procedures to ensure needs are met, including adequate medication/supplies at all times, error/incident tracking abilities.
  • Ensuring the unit is properly staffed and responsible fiscal management is practiced.
  • Serving on an on-call basis, 24 hours a day, to work any shift in the event of scheduling difficulty, crisis, or work stoppage.
  • Overseeing and managing personnel processes pertaining to the selection, orientation, training, supervision, and ongoing evaluation of clinical and education program staff.
  • Reviewing performance measurement data with the director and continually working to collect, analyze, and disseminate program data to improve service quality.
  • Participating on the program leadership team and overseeing the coordination of unit functions.
  • Coordinating closely and collaboratively with representatives of the support functions of PHS including Mission Services, Behavioral Health, Information Technology, Human Resources, and Communications on an as needed basis or per the request of the program director.
  • Attending PHS meetings as appropriate or upon request and participating in work groups and committees organized by senior leadership and other external agencies as appropriate. Attending stakeholder and partner meetings and events as needed.
  • Delivering presentations as needed to enhance program reputation and further integrate the program within the community-at-large.
  • Maintaining accurate, timely, and complete documentation of service.
  • Ensuring all necessary notifications are completed for the unit.
  • Ensuring all information is accurate and updated from intake to discharge for continuity of care.
  • Recording routine case notes and touch points documenting progress and services.
  • Reviewing reports with team to identify service gaps and necessary support services.
