About The Position
NeighborHealth, formerly East Boston Neighborhood Health Center, is a large community health center serving the greater Boston area with a commitment to community health. Every role at NeighborHealth is vital in advancing medicine and delivering the best care experience for patients and the community. This position provides in-home assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to Neighborhood PACE participants. The role requires the ability to work independently in a minimally supervised setting, problem-solve effectively, and show respect for the participant's home and community. Occasional assignments to the PACE Center or other supervised settings may occur.
Requirements
- High School graduate or equivalent required.
- Current certification as a H.H.A. /C.N.A. from an approved program (e.g., Red Cross).
- Must demonstrate competencies before providing participant care.
- A minimum of one year experience working with a frail or elderly population.
- Prior experience as a health aide, personal care assistant, or similar role.
- Excellent customer service and communication skills.
- Physical capability to provide ADL and mobility assistance to all PACE participants.
- Must complete a background check before beginning employment.
Nice To Haves
- Vehicle to travel from home to home preferred.
- Bilingual skills are a plus.
Responsibilities
- Consistently monitors participant ADL/IADL care plans and follows the plan of care.
- Documents care provided on participant flow sheets.
- Checks the health aide voicemail at the beginning of each shift and every two hours, reporting any abnormal findings, alterations in participant status, or care refusal to the team in a timely fashion.
- Assists participants with ADLs per assignment, as defined in the ADL/IADL care plan and as needed.
- Accompanies participants to appointments as assigned.
- Carries out simple treatments under the direction of nursing staff, including simple catheter care and skin assessments during personal care.
