About The Position

The HEAL Program Youth Support Specialist provides outreach, case management, and supportive services to youth ages 14–18 who are at risk of violence, school disengagement, or other barriers to success. Reporting to the Director of Youth Support Services, this position works collaboratively with youth, families, schools, and community partners to connect participants with resources, promote positive youth development, and support safe, healthy, and successful outcomes.

Requirements

  • High school diploma or GED required.
  • Minimum of two (2) years of experience working with adolescents in youth development, education, behavioral health, community outreach, or human services.
  • Knowledge of positive youth development, violence prevention, trauma-informed care, and community resources.
  • Strong communication, organizational, and relationship-building skills.
  • Proficiency in Microsoft Office and electronic documentation systems.
  • Valid Massachusetts driver's license and reliable transportation.

Nice To Haves

  • Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Education, Criminal Justice, or a related field preferred.
  • Experience providing case management or youth outreach services.
  • Experience facilitating youth groups or educational workshops.
  • Training in Motivational Interviewing, Trauma-Informed Care, or Youth Mental Health.
  • Bilingual (English/Spanish).

Responsibilities

  • Conduct outreach, assessments, and case management for youth ages 14–18.
  • Develop individualized service plans that address participants' strengths, needs, and goals.
  • Maintain regular contact with youth and families through meetings, phone calls, home visits, and community outreach.
  • Connect participants to counseling, mentoring, tutoring, workforce development, behavioral health services, substance use prevention programs, and other community resources.
  • Support youth in developing positive decision-making, life skills, and healthy relationships.
  • Deliver violence prevention education, life skills training, and youth development activities.
  • Facilitate workshops, group discussions, and community engagement events.
  • Build collaborative relationships with schools, healthcare providers, community organizations, law enforcement, and social service agencies.
  • Participate in multidisciplinary meetings and community coalitions that support at-risk youth.
  • Represent NNCC and the HEAL Program at community events and outreach activities.
  • Complete participant assessments, case notes, service logs, and required documentation.
  • Maintain accurate participant records and program data in accordance with funding requirements.
  • Assist with program evaluation, data collection, and reporting.
  • Utilize electronic case management systems and Microsoft Office to maintain records and prepare reports.
  • Participate in required trainings, including Trauma-Informed Care, Motivational Interviewing, Positive Youth Development, and violence prevention.
  • Attend staff meetings, community meetings, and professional development activities.
  • Perform other duties as assigned.
