About The Position

Conduit Health is transforming the hardest, most outdated corner of post-acute care: getting essential medical equipment and supplies into patients' homes. We are the first vertically integrated, AI-powered platform that unites ordering, telehealth, prescriptions, insurance, and fulfillment into one seamless experience. In seconds, case managers and providers can say yes to patients who would otherwise wait weeks, while we handle every step behind the scenes. We closed a Series A led by Drive Capital in early 2026 and are scaling fast. The Role We are hiring a Head of Clinical Operations to spearhead strategic initiatives for our fast-growing clinical business, serving as the operational engine for a platform projected to reach eight figures in revenue by year-end. You will own the clinical infrastructure from the ground up, ensuring we scale with the highest standards of compliance, quality, and efficiency. Today, we operate in a high-growth environment where clinical policies and delivery models are ripe for formalization. We need a leader who can build the structural playbook, professionalize our internal operations, and launch critical, high-impact initiatives that leverage our existing patient base. You will define the roadmap for our clinical service delivery and expand the organization that delivers it, improving the systems and operating model needed to scale a sophisticated, internally-managed clinical operation and deliver an exceptional patient experience. This is a hands-on, high-ownership leadership role with full P&L responsibility and a seat at the leadership table from day one. In this role, you will Own our clinical service operations end to end: clinician supply, capacity, scheduling, care delivery performance, and quality across all encounters. Build the recruiting, credentialing, and licensing engine, and scale our clinician workforce to meet aggressive growth targets across multiple states. Develop and oversee clinical quality, including clinical SOPs, documentation standards, supervision structures, and medical-necessity rigor, ensuring consistent, high-quality, and compliant audit-ready care delivery. Design, pilot, and scale our longitudinal care program, including eligibility, consent, matching, time-tracking, and billing. Build and lead key teams: clinical providers, care management, and clinical HR / credentialing. Own the clinical P&L, optimize our staffing models, and transition us from external reliance to a proprietary, high-quality, and cost-efficient operating model. Define and monitor the operational and clinical KPIs that drive quality, access, efficiency, and profitability, using data to continuously improve performance. Join the leadership team and partner across Fulfillment, Patient Experience, Revenue Cycle, Product, and Engineering. About You You are a clinical operator and a builder. You are comfortable being the most senior clinical-ops voice in the room and just as comfortable writing the SOP, auditing a chart, and running a pilot yourself. You are deeply data-driven, using metrics, experimentation, and operational insights to identify opportunities, make decisions, and measure impact. You care about access to care and about unit economics, and you do not see those as in conflict. You can balance operational rigor with innovation, identifying opportunities to rethink workflows, care models, and service delivery as we grow. You might be a fit if you Have 8+ years in clinical operations, including time as the most senior clinical-ops leader or a clear top lieutenant at a growing company. Have built in a startup environment, not just operated inside scale. Have either led provider recruiting at scale or run a high-volume telehealth practice. Have operated inside a real health system, payer, or comparably regulated organization. Have built and scaled a multi-state clinician workforce, including licensing and credentialing. Have stood up clinical quality and compliance programs from scratch: chart audit, training, supervision, SOPs. Are comfortable owning a P&L and thrive in fast-moving, low-ego, ambiguous environments. Bonus points if you have DME / HME, home-based, or post-acute care experience. A clinical license (RN / NP) or clinical training. Hands-on experience with headless EMRs.
