About The Position

Turning Point Family Services is seeking a Qualified Professional for a IIH case manager that will cover case management duties for IIH department. We have been providing mental health services for over 23 years in Iredell, Cabarrus, and Mecklenburg counties. Requirements

  • Minimal requirement for QP is any 4 year degree plus 4 years-experience with the population served or a 4 year degree in human services and 2 years of experience with the population served.

Responsibilities

  • Completing intake and initial PCP’s and crisis plans
  • Submit authorizations to multiple MCO’s
  • Entry portal for all documents needing uploaded into EHR and uploading those documents into the EHR
  • Entering authorizations into OTC
  • Reviewing EHR to ensure intake packet and consents are up-to-date and inform teams when they are needed updated
  • Cleaning up the EHR when warnings display that documents/consents are out-of-date
  • Checking insurances for program monthly to ensure clients insurance are valid and up-to-date
  • Completing audits on cases within the program upon discharge/transition
  • Assisting with tracking of caseloads and authorizations
  • Assisting program with the completion of pre and post outcome measures
  • Responsible for communicating transitions from IIH to transitional services
  • Complete NC TOPPS according to timeframe set by the state
  • Placement search for higher level of care for program and complete necessary documentation in timely manner
  • Train staff on QP responsibilities of updating PCP, writing justification for continued stay criteria, submitting authorization requests, transition forms as well as other functions of the QP as needed
  • Responsive and understands the nature of IIH and the high-paced rate
