About The Position

Youth Haven Services is a comprehensive mental health agency specializing in children, adolescents, and their families. We currently needing two Qualified Professionals to join our Intensive In-Home Program. We are looking for candidates who can serve clients in the counties listed: Persons, Alamance, and Caswell - see below for qualifications about the position. This position will focus on providing direct care, supportive counseling, and rehabilitative services for children and adolescent clients who have a mental/behavioral diagnosis; as outlined in their Person-Centered Plan. Additionally, each position will provide interventions that are strength-based, focus on promoting recovery, symptom reduction, increased coping skills and achievement of the highest level of functioning in the community. Duties must be performed according to YHS policies and standards dictated by State guidelines. Position Summary: Intensive In-Home Team Members are responsible for coordinating and implementing person centered planning for all IIH clients. The Qualified Professional is responsible for multiple tasks related to the clinical/therapeutic functions of Intensive In-Home services, including scheduling, providing therapeutic interventions, skill building activities, maintaining clinical records and completing multiple state service definition requirements.

Requirements

  • Minimal requirement is a 4-year college degree in a human service field with 2 years of documented experience with population served (youth and adolescents with mental health diagnoses).
  • Basic academic skills to teach skill building activities document and maintain required records for facility (i.e., daily attendance logs, mileage logs, MAR, etc.).

Responsibilities

  • Complete intake and initial PCP and submit original authorization and reauthorization
  • Ensure clients medical record is up today with intake packet, yearly consents, etc. Audit client files at discharge/transition.
  • Complete any necessary paperwork for client such as pre/post outcomes, NC TOPPS and discharge SAR’s
  • Responsible for searching for placements for clients and completing necessary paperwork to facilitate placement.
  • Assist with tracking caseloads and authorizations. Check insurance monthly to ensure no changes.
  • Other duties as assigned/required
