About The Position

The HBC Interventionist works collaboratively with youth in Dutchess County who have an intellectual or developmental disability and are at risk of separation from their families because of a potential hospitalization or other out-of-home placement. The role supports youth and families in maintaining safety and stability at home, including through safety planning, assessment, parent training, and connection to local resources. The position involves travel to family homes and other community locations for sessions and outreach.

Requirements

• Bachelor’s degree required.
• Valid driver’s license required.
• Strong organizational skills and accurate attention to detail.
• Excellent communication skills.
• Ability to navigate an electronic health record.
• Ability to adapt and work in a fast-paced environment.
• Flexibility to adjust the daily work plan to meet families’ needs.
• Ability to move throughout the workday and follow people served throughout the community.
• Ability to move in tight spaces.

Nice To Haves

• Master’s degree in a human services field preferred.
• Experience with youth with intellectual or developmental disabilities and/or behavioral health diagnoses.
• Bilingual English/Spanish or English/Creole.

Responsibilities

• Work collaboratively with families to create safety plans for youth in the program.
• Assess youth and families’ strengths, needs, and behavioral concerns.
• Provide parents with training and support on keeping their child safe at home.
• Drive to family homes and other community locations for sessions and community outreach.
• Connect families to local resources.
• Help families apply for OPWDD or OMH services.
• Attend CSE meetings and other appointments with families as appropriate.
