About The Position

The Harm Reduction Worker is responsible for helping with the daily operations of Evergreen’s Drop-In Services, including the provision of services in a non-judgmental manner, such as coordinating showers, laundry, food and beverage, and individual and group support. This role ensures that services are provided smoothly, in a safe environment, and are tracked appropriately.

Requirements

• Associate degree in public health or related field, or two years of experience working with at-risk populations
• Ability to travel between office locations as needed
• Have and maintain a current and valid NYS driver’s license, verified thought the NYS LENS system
• Excellent verbal and written communication skills
• Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

Responsibilities

• Provides direct patient services in the Drop-In Center
• Takes initiative on warmly greeting patients, patient intake, hospitality services, coordinating shower and laundry services, and creating, implementing, and facilitating groups
• Documents patient interactions in the electronic health record (EHR) and actively collaborates with other providers and departments within EHS
• Adheres to company policy regarding documentation standards and completion timeframes
• Assists patients in identifying personal goals and follows up regularly to assess readiness to address their stated goals
