The Department of Economic Security (DES), Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Habilitation Nurse II. This position is responsible for understanding the physical health care needs and coordinating care for members with intellectual/developmental disabilities (I/DD) who have skilled nursing needs. This position provides specialized nursing case management for individuals with intellectual/developmental disabilities (I/DD) requiring skilled nursing care; completes required documentation, reporting, and recommended health care prevention plans; ensures physicians' orders are obtained, updated, and renewed with therapeutic interventions per contractual agreement requirements; acts as a consultant to individuals, families and staff regarding treatment of member illness, rehabilitation needs, and preventative care; participates as a member of interdisciplinary teams; and represents the medical component during quarterly Person Centered Support Plan (PCSP) meetings. Flexibility in schedule is highly desired as these are 24 hour residential settings and periodic monitoring is required. Must be willing to work weekends, holidays, nights and overtime, when necessary. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community. Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.
