About The Position

This position is responsible for providing Habilitation Coordination to individuals with intellectual and developmental disabilities living in nursing facilities and have a working knowledge of all TAC and HHS PASRR program requirements. This includes, but not limited to, TMHP alerts, PASRR Evaluations, maintaining required monthly contacts, linking to communities and resources, coordinating and monitoring services provided by the NF and Local IDD Authority, and advocating in accordance with the philosophy, goals, and objectives of Texoma Community Center. Participates in fidelity reviews as assigned and operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.

Requirements

Bachelor’s Degree in Sociology, Social Work, Psychology, Education or Counselling as outlined in TAC.
Current driver’s license and vehicle for use at work.
Excellent communication skills (verbal & written) and the ability to interact with diverse populations are essential.
Knowledge of Microsoft Office Suite.
Consistently practices Trauma-informed Model of Care, understands and considers the pervasive nature of trauma.
The knowledge of cultural differences and ability to work with individuals from different cultural backgrounds.
Ability to generate, read, interpret, and take action as it relates to basic financial statements, national and state regulations, operational and maintenance and organizational policy / procedures.
Ability to effectively communicate in both a written and verbal format.
Ability to work cooperatively and communicate effectively to maintain good working relationships.
Ability to work with skill in identifying problems.
Will have access to confidential information abiding by the organization’s privacy policies and regulations concerning this information.

Nice To Haves

One year experience working directly with persons with an intellectual disability or related conditions preferred.
Ability to speak a second language relevant to population served is preferred.
