About The Position

The mission of On Your Mark is to provide innovative and comprehensive community-based service to people with intellectual and developmental disabilities across their lifespan. The Day Habilitation Coordinator is responsible for the daily oversight and supervision of assigned program areas and staff. This position designs and implements curricula for individuals with Autism and Intellectual Disabilities, ensuring that activities align with each participant’s goals and abilities. The coordinator assists in developing lesson plans, creating Day Habilitation Plans, Staff Action Plans (Initial 30-Day, Semi-Annual, and Annual), and preparing all required billing documentation. The role includes representing the program at annual and semi-annual Life Plan meetings, managing correspondence related to assigned individuals, and coordinating all community-based instructional and recreational activities. The coordinator fosters volunteer opportunities and ensures professional communication with caregivers, Care Managers, and other professionals. In collaboration with Directors, the Coordinator supports program initiatives such as curriculum development, admissions, group placement, transfers, and staff development. This position also requires providing personal care support to participants, including toileting assistance, mobility support (e.g., gait belts, walkers, wheelchairs), and behavioral intervention utilizing SCIP-R techniques as needed during crisis situations.

Requirements

  • Strong knowledge of intellectual and developmental disabilities, individual differences, and OPWDD regulations.
  • Minimum two years of supervisory experience in the developmental disabilities field.
  • Ability to instruct and support program activities such as physical fitness, cooking, academics, crafts, horticulture, cognitive skill development, music therapy, and community integration.
  • Excellent organizational, interpersonal, and communication skills.
  • Strong written communication and documentation abilities.
  • Ability to identify changes in participant behavior or condition, determine interventions, and act promptly.
  • Skill in identifying staff training needs and promoting professional development.
  • Self-directed, flexible, and motivated, with the ability to manage responsibilities independently while adapting to changing participant needs.
  • High School Diploma or GED required.
  • Minimum one (1) year of experience working with individuals with Autism and Intellectual Disabilities.
  • Valid Driver’s License.

Responsibilities

  • Immediately report any incidents or suspected incidents of abuse and/or neglect to a supervisor.
  • Ensure program compliance with all OPWDD and New York State Justice Center regulations.
  • Collaborate with staff to develop program schedules and activities that promote participation in areas such as Fitness and Wellness, Independent Living, Music Therapy, Art Therapy, Aquatics, Computer Literacy, Academics, Cooking, Horticulture, and Communication Skills.
  • Provide on-site leadership and supervision to Day Habilitation staff, ensuring high-quality programming and adherence to activity schedules.
  • Oversee the implementation of all program activities and provide direct guidance on Staff Action Plan goals and valued outcomes.
  • Conduct regular team meetings to provide supervision, leadership, and direction.
  • Monitor staff performance, utilize staffing resources efficiently, and adjust assignments as needed.
  • Support participants in achieving their fullest potential and maintaining positive behaviors and attitudes.
  • Serve as an active member of the interdisciplinary team.
  • Conduct assessments for new admissions and ongoing participant evaluations.
  • Review and submit billing documentation on the 15th (or preceding Friday) and last day of each month.
  • Document and report participant progress to the appropriate supervisory or clinical staff.
  • Provide input on program design and modification based on participants’ evolving needs.
  • Assist with transportation as needed and support arrival and departure routines.
  • Develop staffing schedules to ensure adequate coverage for personal care and medication administration (as applicable, following AMAP certification).
  • Attend all required trainings and certifications, including OPWDD in-service sessions, AMAP, CPR, First Aid, and SCIP-R.
  • Notify supervisors in advance of absences or lateness to ensure appropriate coverage and participant safety.
  • Maintain program supplies and ensure all areas are clean, organized, and properly equipped.
  • Respond promptly and appropriately in emergency situations.
  • Assist in supervising participants during meals.
  • Perform other duties as assigned to ensure program safety, quality, and compliance.
