About The Position

Family Service of Roanoke Valley is hiring for our Private Guardianship Program. The Guardian Representative is the decision maker for some of our most vulnerable adults in the community. These individuals having differing intellectual abilities are in need of someone willing to serve as their decision maker to ensure they are living their best life possible.

Requirements

• A combination of education and experience equivalent to a bachelor's degree in a related area is required.
• Excellent case management skills are a must.
• This position is also required to follow all Code of Virginia regulations regarding guardianship services.

Nice To Haves

• A bachelor's degree in in a related area such as social work, mental health, nursing or other human service program is preferred.

Responsibilities

• Engaging successfully with an interactive team
• Make informed decisions on behalf of the individual including residential, medical and other appropriate services
• Honor the wishes of the individual to the greatest extent possible
• Monitor the quality and appropriateness of services by attending medical appointments and care planning meetings
• Maintain accurate and current records, including an Electronic Health Record for each individual served
• Share on-call responsibilities
• Manage daily and monthly scheduling to ensure quality service delivery.
