About The Position

Lutheran Social Service of Minnesota is a dynamic social service organization dedicated to serving children, youth and families, people with disabilities, older adults, and military veterans. As a Guardian Conservator, the role involves carrying out the duties of a court-appointed Guardian/Conservator in accordance with MN statutes. This includes providing financial management and/or ensuring care for vulnerable adults within the Guardianship Options Program. A Guardian Conservator will manage a caseload of 35-42 individuals, typically covering a specific geographic region of the state.

Requirements

• Two years of experience with similar client populations and needs.
• Valid driver's license.
• Previous lived experience in making clear and unbiased thinking and decision-making skills.
• Ability to exercise sound judgement regarding best interest of client, major financial transactions, and life & death situations.
• Extreme detail attention to communications, accurate and timely documentation, and billings.
• Must be cleared by a Bureau of Criminal Affairs (BCA) background study.
• Complete a Motor Vehicle Check (MVR).
• Employer-sponsored accreditation through the Center for Guardianship Certification (CGC) - must be obtained after one year of employment.

Responsibilities

• Advocate for individuals for least restrictive life choices that enable them to live full and abundant lives.
• Work with individuals and families in the development and service delivery of supported decision-making.
• Work closely with courts, including annual reports.
• Supervise and monitor client personal and health care. This can include authorizing medical, therapy, treatment, support, vocational, and other services.
• Sign legal documents - such as consents, admissions, and lease agreements.
• Work as a member of a support team to ensure appropriate living environment and services.
• Travel to visit clients at least monthly.
• Provide substitute decision-making regarding medical emergencies and other sudden changes in client's situation.
• Be responsible for the financial management and oversight of individuals assigned to them.
• Work with financial team to oversee financial assets.
• Be a member of an on-call schedule for client emergencies.
