About The Position

Quest, Inc. is seeking a Group Home Medical Coordinator to support individuals with developmental disabilities living in our residential group homes. This role serves as a vital link between healthcare providers, pharmacies, residential teams, and the individuals we serve, ensuring that medical needs are coordinated effectively and accurately. This position will work onsite, primarily supporting 2–3 group homes and about 26 individuals while partnering closely with residential staff and healthcare providers. This position reports to the Group Home Medical Case Manager and is scheduled to work Monday through Friday, from 7:00 AM to 3:00 PM.

Requirements

  • High School Diploma or GED.
  • Two or more years of experience in healthcare coordination, medical records, residential services, caregiving, case management, or a related healthcare/support services field.
  • Experience coordinating appointments, medical documentation, or healthcare services.
  • Valid Florida Driver's License with an acceptable driving record.
  • Ability to drive company vans and other passenger vehicles.
  • Strong organizational, communication, and time-management skills.
  • Basic computer skills and experience maintaining accurate documentation.
  • Ability to work independently while collaborating with multiple teams.
  • Ability to pass a Level II Background Screening and meet all agency requirements.

  • Experience supporting individuals with developmental disabilities or other vulnerable populations.
  • Medication administration experience.

Responsibilities

  • Coordinate and maintain medical care for individuals residing in Quest group homes.
  • Ensure medications, physician orders, medication administration records (MARs), and medical documentation are accurate and up to date.
  • Schedule, coordinate, and track medical appointments, including primary care, psychiatry, neurology, podiatry, and specialty visits.
  • Serve as a liaison between healthcare providers, pharmacies, residential staff, and families/guardians as needed.
  • Complete medical documentation and ensure medical information is properly maintained within agency systems.
  • Monitor medication inventories and coordinate prescription refills.
  • Accompany individuals to medical appointments when necessary and ensure appropriate follow-up care.
  • Train, support, and provide guidance to direct care staff regarding medical procedures and documentation requirements.
  • Administer medications and ensure compliance with agency and regulatory requirements.
  • Assist residential teams with direct support responsibilities when operational needs require additional coverage.
  • Drive agency vehicles to transport individuals to appointments and community activities.
  • Collaborate with the Group Home Medical Case Manager to support quality healthcare outcomes for the individuals we serve.
