As a Primary Care Physician, you will serve as the cornerstone of care for an established panel of adult and geriatric patients. Working alongside a dedicated interdisciplinary care team, you will focus on improving health outcomes, preventing hospitalizations, and helping patients successfully manage chronic conditions. Common conditions managed include: Diabetes Hypertension Congestive heart failure Coronary artery disease COPD and respiratory disorders Chronic kidney disease Arthritis and mobility limitations Multiple chronic comorbidities common within the Medicare population This role is ideal for a physician who values continuity of care, patient education, and long-term relationships.

Requirements

  • MD or DO degree from an accredited medical school
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine
  • Active Florida medical license or ability to obtain
  • DEA certification
  • Commitment to high-quality, patient-centered care

Nice To Haves

  • Experience caring for Medicare and senior populations
  • Chronic disease management experience
  • Value-based care experience
  • Bilingual English/Spanish skills a plus

Responsibilities

  • Provide comprehensive primary care services for adult and geriatric patients
  • Diagnose, treat, and manage acute and chronic medical conditions

  • Coordinate care with specialists, hospitals, and ancillary providers
