The Gap Services Case Manager provides appropriate case management and supportive gap services to eligible Cuban/Haitian Entrants (CHE) and other ORR-eligible populations through outreach, intake and assessment, referral, and follow up. The CHE case manager will provide individualized health education and advocacy for the CHE’s access to healthcare and necessary referral services to help CHE clients become more socially integrated and culturally acclimated in their new communities. The CHE case manager will provide support to all CHE’s for a maximum of 90 days unless a client is especially vulnerable. In the event of a an especially vulnerable client in need of services beyond the 90-day service period, (large family, women-at-risk, elderly clients, CHE’s that identify as LGBTQ, CHE’s with significant physical or mental disabilities, and other vulnerable populations) the CHE case manager will refer clients to Intensive Case Management (ICM) for continued appropriate services.
