About The Position

This position oversees and manages the authorization and utilization of services delivered for families determined by the Department of Children and Families to be at high or very high risk. The Care Coordinator coordinates and schedules Family Team Conferences to promote healthy, stable families with the goal of preventing further involvement with the system of care and keeping families intact. The Care Coordinator maintains information regarding the environment of current services to ensure a flexible, responsive, culturally competent service delivery system.

Requirements

  • Valid Driver's License: Required to travel throughout the County.
  • Computer skills (Microsoft and Web based application): For daily entry of case activity and utilization management of service authorizations.
  • 2+ years of Team Facilitation Skills with At Risk Families: Ability to effectively partner with collaterals and service providers.
  • Min 1 year knowledge Child Welfare Experience.
  • Bachelor’s Degree required.
  • 2 to 5 years related experience required.

Nice To Haves

  • Master’s Degree in Human Services Related field preferred.
  • 5 to 8 years related experience preferred.

Responsibilities

  • Authorization Coordination: Manage budget allocations, identify alternative community resources and funding streams, and adhere to policies and procedures. Responsible for utilization and monitoring flexible funding and service coordination to meet client needs.
  • Care Planning Resources: Access 3rd party, Medicaid, and community resources and ensure inclusion in care planning.
  • Crisis Interventions: Provide crisis intervention linkages, referrals, and consultation to ensure family support and stabilization. Follow up with emergency FTC for continued authorization. Ensure interim safety plan and follow-up development.
  • Facilitate FTC's: Bring together professionals, family, and community resources to support the family, develop strategies, and identify needs. Use judgment and a solution-focused style to address differing points of view and achieve consensus. Ensure the family voice drives the process and decisions have group support.
  • Service Utilization UM/Wrap: Manage the daily service utilization log and submit to C.A.R.E.S. FSS Supervisor for authorization. Monitor costs and track other sources of services and funding. Coordinate with providers to ensure timely service delivery and that needs are addressed as detailed in individualized care plans.
  • Other duties as required: Fulfill incidental or related duties, assist and train others, and perform duties of higher-rated positions for developmental purposes.
