Join our close-knit team of case managers to make a difference in the lives of low-income children/youth and their families, receiving mental health services in the San Fernando Valley. We are seeking a supportive, creative, and resourceful case manager as an essential member of our clinical team to improve outcomes in the areas of access to community resources that can address family needs and provide support. Intensive orientation, ongoing weekly (1-1.5 hours) supervision, hands-on-training and a small caseload are integral features of the job. Case managers work hand-in-hand with all members of the client's mental health team to develop/monitor a treatment plan in addition to collaborating with staff at local public schools to advocate for appropriate educational services that supports school success. Witnessing our families improve their lives is the most rewarding aspects of this position. We are seeking a full-time Bilingual Case Manager for the Full-Service Partnership (FSP) program. This position reports to the Community Resource Services Director. The FSP Case Manager is primarily responsible for providing in-home case management services to families receiving mental health treatment through the Northridge-based Full Services Partnership Program.
