About The Position

The Foster Care Placement Coordinator coordinates foster care home certification activities and supports the placement of children into approved foster care and residential settings. The role works closely with the Program Manager and Foster Care team to support daily operations, ensure compliance with regulatory requirements, and maintain effective communication throughout the placement process. Requirements

  • Bachelor’s degree in social or behavioral sciences.
  • One year of experience coordinating foster care, licensure, or child welfare–related documentation in a human services setting.
  • Must be at least 21 years old.
  • Ability to maintain strict confidentiality with all records and information.
  • The ability to effectively communicate verbally and in written format in both individual and group settings.
  • Ability to be flexible, and able to change direction rapidly.
  • Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed.
  • Basic computer knowledge.
  • Ability to anticipate, conceptualize and communicate an individual’s unique needs with the interdisciplinary team.
  • Exercise independent judgment within the scope and boundaries of the department, the Agency’s Standard Operating Procedures, and Federal, State and Local Rules and Regulations regarding department operations.
  • Knowledge of human development regarding major milestones of development, and normal development parameters.
  • Ability to provide reliable, insured transportation for self and others through the community served.

  • Current Structured Analysis Family Evaluation (SAFE) Certification.
  • One year of experience communicating and coordinating with internal teams and external agencies (CDHS, providers, or licensing entities) to support placements or case related activities.
  • One year experience maintaining accurate and timely case records, tracking deadlines, certification status and required documentation for certification of Foster Homes.
  • Structured Analysis Family Evaluation (SAFE) Certification at next available opportunity.

Responsibilities

  • Coordinate foster care home certification and placement activities in accordance with federal, state, county, and agency policies, rules, and regulations, with a focus on child safety and compliance.
  • Complete, coordinate, and review Structured Analysis Family Evaluation (SAFE) home studies and annual updates as part of the foster parent licensure process.
  • Support placement matching by coordinating information among foster homes, case managers, and referral agencies to meet individual child placement needs.
  • Assist with communication and coordination with the Department of Human Services and other collaborative agencies, maintaining positive and professional working relationships.
  • Support the recruitment, screening, coordination, and scheduling of training activities for prospective foster families.
  • Assist with documentation and communication related to foster care provider and case management rates, as directed by the Program Manager.
  • Participate in ongoing program evaluation activities by providing input, data, and feedback to support program effectiveness and improvement efforts.
  • Provide support to case managers by coordinating information, documentation, and resources as needed.
  • Navigate multiple stakeholders to support timely and appropriate placements while adhering to regulatory requirements.
  • Maintain working knowledge of Volume VII rules and regulations, HCPF program funding streams, and foster care placement requirements relevant to assigned duties.
  • Perform case management related duties as assigned and within scope to support program operations.
  • Must work closely with other Ariel Programs and offices.
  • Performs other duties as assigned.
