About The Position

Case Manager Office located in Mt. Laurel Service areas (Burlington, Gloucester, Mercer counties) $45,000 Case Manager Everybody needs a job but only extraordinary people work here. Utilize Microsoft Windows programs to track and maintain data for multiple rolling deadlines related youth in care Communicate with external team partners including DCPP, CMO, and family members of youthConduct home visits and office base services, and accompany individuals being served as neededResponsible for the annual contractual review process for Host Home Providers Maintain and audit files regularly for compliance Coordinate care with team members to ensure youth receive high-quality careExceptional organization and time management practices required Flexible in daily schedule to best facilitate program needsInterface effectively with internal team members to ensure licensing regulations are met Performs other related duties and activities as required

Requirements

  • Bachelor’s degree in human services.
  • Three years of related work experience and knowledge of case management.
  • Valid driver’s license, registration, and insurance.
  • Ability to establish working relationships with individuals served.
  • Demonstrated competence in verbal and written communication skills.
  • Ability to handle crisis situations.
  • Working knowledge of Excel
  • Ability to learn other relevant computer systems
  • Ability to work independently

Responsibilities
