About The Position

Clatsop Behavioral Healthcare (CBH) has an immediate opening for a full time Forensic Case Manager. This position provides intensive case management services to incarcerated individuals ordered to participate in the Aid & Assist program. This role focuses on legal coordination, clinical stabilization, and transition planning within the correctional environment. This role also serves the purpose of discharge-planning for non-Forensic individuals struggling with acute mental health symptoms and/or Severe and Persistent Mental Illness (SPMI). This position receives day-to-day direction and coordination from the Forensic Clinician to ensure alignment of forensic services across community and jail-based settings, and reports directly to the Crisis Systems Program Manager. This position also collaborates closely with other jail-based positions for the purposes of release planning and completing referrals to post-release residential treatment.

Requirements

  • Active certification as a Qualified Mental Health Associate (QMHA) and Certified Alcohol and Drug Counselor (CADC) in the State of Oregon
  • Knowledge or ability to learn about Medicaid, Oregon Health Authority (OHA), and related State and Federal health care regulations to include Oregon Administrative Rules (OARs) and statutes
  • Excellent oral and written communication skills
  • Strong attention to detail, highly organized
  • Knowledge of the Health Insurance Portability and Accountability Act (HIPAA) and the importance of maintaining confidentiality
  • Ability to pass a criminal background check
  • Have and maintain an Oregon or Washington Driver’s license and meet the agency’s insurance standards

Responsibilities

  • Participates in daily Crisis Team meeting
  • Supports community-based Mobile Crisis Case Management (QMHA) interventions as needed
  • Attends weekly Court hearings, conducts weekly Legal Skills groups, and submits weekly status reports for the Aid & Assist caseload.
  • Regularly checks all pertinent cases in the Oregon Judicial Department website to ensure up-to-date knowledge.
  • Participates in investigations, hearings, and legal proceedings as required.
  • Assists with referral and coordination of entry into diversion programs such as Mental Health Court or Drug Treatment Court as needed.
  • Participates in ongoing case management, needs assessments, and release planning
  • Provides follow-ups to incarcerated individuals identified by Mobile Crisis
  • Identifies acutely symptomatic incarcerated individuals needing additional support and communicates needs to appropriate supports
  • Supports incarcerated individuals not ordered to Aid & Assist to minimize recidivism and divert from future incarcerations
  • Completes referrals for post-release Residential SUD and MH treatment.
  • Participates in jail discharge planning as needed in support of conditional release requirements for individuals ordered into the Aid & Assist program and/or the SPMI population, collaborating with other CBH jail-based positions
  • Connects non-Forensic individuals to appropriate post-release community supports and resources.
  • Collaboratively manages a variable caseload of clients involved or at risk of involvement in the legal system.
  • Changes method and degree of treatment when indicated by client reactions
  • Encourages use of coping skills; facilitates Legal Skills or other training groups; teaches life skills.
  • Refers clients to other supportive services to supplement treatment.
  • Regular consultation with other Crisis and jail-based team members to ensure client needs are being met.
  • Assists with referral and coordination of entry into other programs within CBH as needed
  • Documents case updates for Aid & Assist and other cases in CareLogic and shares information with all parties concerned
  • Documents all services according to agency standards and timelines
  • Data collection for the incarcerated population as needed.
  • Coordinates with CBH program administrators to provide accurate updates on client engagement
  • Coordinates with ENCC and ensures timely recording of data on aid/assist report
  • Tracks and collects client caseload data in real-time for CFAA reporting purposes
  • Performs outreach and engagement as a preventative measure (i.e., Forensic Diversion)
  • Collaborates with natural and community supports
  • Consults and coordinates with community partners
  • Coordinates with outreach workers for community engagement
  • Additionally provides in-person community outreach to clients and connection to natural supports
  • Obtains Releases of Information to coordinate care
  • Provides services with a collaborative, solution-focused approach
  • Maintains confidentiality and ethical standards
  • Remains flexible to system demands
  • Other duties as assigned
