About The Position

EAC Network, a leading not-for-profit social service agency, is seeking a dedicated full-time Forensic Case Manager to join the Staten Island TASC Program. This role involves supporting the Behavioral Health & Justice Involved Population by providing screening, treatment planning, case management, and court reporting for clients with substance abuse and/or mental health issues. The agency serves over 80,000 people through 100 programs across Long Island and NYC.

Requirements

  • Associate or Bachelors degree in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology and/or a significant combination of education and experience in either mental health, substance abuse or criminal justice.
  • Good communication skills, both oral and written.
  • Ability to work cooperatively with treatment and criminal justice systems.
  • Proficient in computer applications such as spreadsheets, Microsoft Office applications, calendar, email and/or database software.

Responsibilities

  • Act as a liaison to the courts and correctional facilities.
  • Provide screening services for clients involved with alcohol or drug abuse and/or presenting evidence of mental illness to ascertain appropriate treatment.
  • Effectuate referrals to mental health, drug abuse, or alcoholism facilities and arrange transportation for clients to the appropriate facility when necessary.
  • Assist clients in applying for and receiving public and private benefits.
  • Maintain case documentation, including client records, progress notes, and other relevant information.
  • Monitor client participation in treatment programs through case management appointments, progress reports, and conferencing with treatment personnel.
  • Prepare and present written and oral status reports to the criminal justice system regarding client progress.
  • Investigate and recommend supportive services for clients.
  • Conduct drug testing on clients as needed.
  • Perform timely visits to treatment and correctional facilities.
  • Perform all other relevant duties as determined by Supervision and EAC Network Administration.
