About The Position

Covenant Village is seeking a full-time Day Shift Floor Nurse to join their compassionate team. This role involves performing and managing diversified duties in providing resident care, with responsibilities including managing CNA workload, providing guidance and support to nursing assistants, performing head-to-toe assessments, identifying and responding to changes in resident condition, developing care plans, monitoring resident well-being, administering medications and treatments, providing wound care, and ensuring accurate and timely documentation. The nurse will also communicate with physicians and families, carry out restorative programs, and ensure compliance with regulations. The position requires a valid North Carolina nurses license (LPN or RN) and experience with the senior population is preferred. Excellent communication, organizational, and leadership skills are essential, along with the ability to work independently and collaboratively. The role demands patience, empathy, adaptability, and attention to detail. Physical requirements include prolonged periods of sitting, standing, walking, and the ability to physically assist residents, lift up to 45 pounds, and perform various physical movements. Individuals may be exposed to various conditions and hazardous materials, with appropriate training and protective devices provided.

Requirements

  • College or Nursing school diploma/degree.
  • Valid North Carolina nurses license (LPN) or (RN).
  • Capable of adequately managing resident care responsibilities, as well as personnel and administrative responsibilities at the direction of the Director of Nursing and the associated nursing management.
  • Excellent communication, resident relations, organizational skills, customer service orientation, and leadership ability.
  • Ability to work independently with initiative and as a collaborative and supportive member of a team.
  • Ability to act with integrity, professionalism, and confidentiality.

Responsibilities

  • Assigns and supervises CNA workload and resident assignments.
  • Provides guidance, coaching, and support to nursing assistants.
  • Addresses performance concerns and reports to Nursing management as needed.
  • Performs, supervises, and assists with the completion of ADLs, rounds, and safety checks to ensure proper nursing care.
  • Performs head-to-toe assessments and ongoing monitoring of resident conditions.
  • Identifies and responds to changes in condition; initiates appropriate interventions.
  • Evaluates resident needs and conditions to assist in the development of individual care plans.
  • Reviews and follows resident care plans with physician, MDS coordinator, or other interdisciplinary team members, and updates nursing notes accordingly.
  • Monitors skin integrity, pain, nutrition, hydration, and mobility status.
  • Receives and transcribes physicians’ orders timely and accurately.
  • Administers medications and treatments per physician orders and MAR/TAR.
  • Performs wound care, injections, tube feedings, catheter care, and other skilled tasks within scope.
  • Monitors for medication effectiveness and adverse reactions.
  • Performs other professional services as ordered and required.
  • Provides clear shift report and documentation accurately and timely to ensure continuity of care.
  • Gives emergency treatment when required and notifies the physician of an emergency and carries out physician’s orders.
  • Identifies safety hazards and initiates corrective action.
  • Communicates with physicians, pharmacies, labs, families, and interdisciplinary team via phone, text, or other designated means.
  • Carries out restorative and rehabilitative programs for residents.
  • Conducts resident rounds as assigned to assess resident issues and reports them to the charge nurse, nursing supervisor, or D.O.N. and takes the necessary follow-up action as directed.
  • Attempts to fulfill spiritual and psychological needs of residents delegating to appropriate personnel when necessary.
  • Directs and coordinates department cleaning and care of equipment with maintenance and housekeeping.
  • Attends mandatory nursing meetings, completes education as assigned by the due date, and any in-services or trainings as directed.
  • Assumes responsibility for compliance with Federal, State, and local regulations as it pertains to Long Term Care.
  • Assists with orientation, instruction, and supervision of other nursing personnel.
  • Completes incident/accident reports and initiate investigations per policy.
  • Implements fall prevention, elopement, and safety procedures.
  • Admits and orients new residents and relatives.
  • Checks identification and reviews resident’s medical history.
  • Checks physician’s orders and pre-admission information.
  • Arranges for diagnostic and therapeutic services as ordered by resident’s physician.
  • Orders prescribed medications, supplies, and equipment.
  • Sends diet orders to the food service department.
  • Submits all admission forms to the RN for review and signature.
  • Ensures resident documents are scanned and organized in the chart according to policy.
  • Completes assigned resident monthly summary forms.
  • Follows standard precautions, hand hygiene, injection safety, and PPE use per infection control policy.
  • Requisitions supplies, equipment, linens, emergency drugs, etc., as needed.
  • Maintains all necessary records including controlled drug records.
  • Completes medical chart forms, as necessary.
  • Upholds HIPAA and resident rights in all circumstances.
  • Maintains LPN/RN licensure in good standing, retains CPR BLS certification, and participates in continuing education relating to the long-term care field of nursing.
  • Ensures clean work environment of nursing station and cart including stocking, removing expired/discontinued medications, and keeping counters free of clutter.
  • Maintains confidentiality and respect for residents and employee privacy.
